For instance, lack of environmental stimulation significantly disrupts dendritic growth (Kolb and Fantie 1989; Kolb and Gibb 1999). Neuronal differentiation is also important for a number of formative processes, including the later formation of neurotransmitters and specific receptor cells at postsynaptic sites. Each differentiating neuron must distinguish which neurotransmitters it will ultimately use. Similarly, postsynaptic sites must express the receptors they will need to receive input from presynaptic sites.
As an example, one study of very low birth weight children born between 1947 and 1950 found that 44 percent of these children had intelligence quotients under 70, and only one-third escaped any disability (Lubchenco, et al. 1963). The outcomes were better for moderately low birth weight infants (1,500 to 2,499 grams), but still worse than for their normal birth weight counterparts. During this time period, children born below 1,000 grams (or less than 28 weeks gestation) usually died. If they survived, these children experienced higher levels of physical, psychological and cognitive disability (Hack, Klein and Taylor 1995).
The purpose of this chapter is to present an overview of the LBW outcomes in relation to intelligence, specific cognitive abilities, behavior and academic performance. In addition, this chapter will discuss the pathophysiological mechanisms by which LBW relates to such outcomes. Prior to embarking on such review, it is important to define LBW and distinguish it from preterm birth and intrauterine growth restriction (IUGR). Definitions The World Health Organization (WHO) has defined low birth weight (LBW) as a weight at birth of less than 2,500 grams (WHO 1980).
